99 Explorer w/ P0174, P0171, P1151, P1131 DTCs

I have a '99 explorer w/ 4.0 engine. It runs rough whenever the engine is under a load. It is showing the following codes: P0174, P0171, P1151, and P1131. It has had the intake manifold gaskets replaced within the last 1000 miles because it was doing the same thing then as it is now. The freeze frame is showing all the fuel trims at 25% and a CL fault on fuel sys 2. I would appreciate any ideas on where to look to find the problem.

check for engine vacuum leaks- might want to start checking PCV and related hoses a known issue... if no vacuum leaks check fuel pressure with a gauge = let us know.

The fuel pump is relatively new. It was replaced 4 months ago and it has about 10,000 miles on it. I will check the pcv and hoses. Thank you for the help.
