2011 Ford F250 Diesel P2456 Code Reappears After Clearing - DPF Related?

I own a 2011 Ford F250 6.7L diesel with both K&N air filter and the P2456 diagnostic code frequently reappearing. I use a code reader to clear the fault, but it returns within one to two days. Occasionally, during acceleration, the truck feels like it loses power. Is this a Diesel Particulate Filter (DPF) issue? If so, what steps should I take to diagnose and resolve it?
