P0401 indicates insufficient EGR (Exhaust Gas Recirculation) flow. Possible causes include a faulty EGR valve, loss of vacuum source, or clogged EGR passages. To diagnose: remove the EGR valve and observe engine behavior. If the engine fails to idle or runs roughly, the passages are likely open; if it idles smoothly, the passages may be clogged. Also inspect the vacuum line connected to the valve for cracks or breaks. If you have a vacuum pump, apply vacuum to the valve while the engine is running—properly functioning valves will cause the engine to stall or nearly stall.
