2005 Dodge Stratus 2.7L 6-Cylinder P0133, P0138, and P2096 Codes: One Issue or Multiple Problems?

I own a 2005 Dodge Stratus with a 2.7-liter 6-cylinder engine. I'm currently experiencing three persistent OBD2 diagnostic codes: P0133 (O2 Sensor 1/1 Slow Response), P0138 (O2 Circuit High Voltage), and P2096 (Downstream Fuel System Lean Bank 2). My question is whether these codes point to a single underlying issue or multiple separate problems. I don't notice any loss of power, and the catalytic converter doesn't appear to be overheating, which suggests no major exhaust blockage. Given that many Chrysler vehicles share similar issues, I'm wondering if this combination of codes is common and what steps should be taken to diagnose and resolve it.

There is no visible exhaust leak—no components have been removed. To properly assess the issue, use an OBD2 scanner or a digital voltmeter (DVOM) to measure the actual return voltage from the oxygen sensor. Possible causes include: O2 sensor voltage above 3.7 volts; O2 sensor signal circuit shorted to power; O2 sensor return circuit shorted to power; O2 sensor signal circuit open; or O2 sensor return circuit open.

I replaced the bank 1 oxygen sensor, which caused the P0133 and P2096 codes to appear. I then swapped the bank 1 sensor with the bank 2 sensor to see if the code would transfer—however, the same codes persisted on the original bank. I've checked for exhaust leaks (none found), inspected wiring, and confirmed they are intact. After replacing the manifold gaskets, the codes remain unchanged. I'm now at a loss as to what's causing this issue.

What is the actual voltage reading at the oxygen sensor? Did you install an OEM-quality sensor rather than a generic aftermarket one? Can you use a digital voltmeter (DVOM)? Do you have access to the correct wiring diagram to verify proper connections?

My 2003 PT Cruiser turbo is showing P0352—after replacing the ignition coil, spark plugs, camshaft position sensor, and crankshaft position sensor, it still misfires on cylinders 2 and 3. Anyone have insight before I continue replacing components?

The P2096 code specifically indicates a fuel system circuit issue—check whether your vehicle has an '8' in the 8th character of the VIN, which may indicate a G-series engine with only one coil having four secondary posts. This is a waste spark ignition system where cylinders 2 and 3 are paired. Confirm wiring at the coil: Is there 12V on the 1st and 3rd signal circuits? Are they open, shorted to ground, shorted to 12V, or not being driven properly (on/off)? Power is supplied via DKGRN/ORG; the other two wires are driver signals—these should flash when cranking or running.
