It happened to me just yesterday!!!! At all Tesla chargers, about 200–300 times. It was the first time it ever happened to me, and I got really scared, thinking the car had broken down.

Only once using the travel charger, of course when I was 200 miles away from the car. In the end, the charger just needed to be taken out and reseated.
